Disabled American Veteran 100% - DEERS Enrollment

To enroll a 100% Disabled Veteran and eligible family members in DEERS, the following documents are required: 

  1. Veteran’s Eligibility Letter from the Veterans Affairs showing eligibility. The letter MUST read that the veteran is 100% permanently disabled, unemployable, or is 100% temporarily disabled WITH a reevaluation date.  These veterans are eligible for Commissary, Exchange, and Morale, Welfare, and Recreation (MWR) privileges. 
  2. Two (2) valid, unexpired, forms of identification listed in the Federal Information Processing Standards 201-2, and one form MUST be a photo ID. 
  3. Veteran’s DD Form 214 (Member 4 copy) to verify discharge and service information.
  4. Veteran’s Birth Certificate (Original copy with file number). 5. Veteran’s Social Security Card (Original, not laminated).

*Note* The Veteran and eligible dependents receive an ID card for Commissary, Exchange, and Morale, Welfare, and Recreation (MWR) privileges ONLY. 

 *Note* The Veteran and eligible dependents are NOT entitled to Tricare/Medical privileges. The Veteran and eligible dependents are eligible to receive medical benefits through the Department of Veteran Affairs Civilian Health and Medical Program.

Spouse - DEERS Enrollment

To enroll a spouse in DEERS, the following documents are required: 

  1. Sponsor must be present OR a Power of Attorney must be presented and signed by the sponsor OR sponsor may nominate dependent on the Rapids Self-Service website: https://www.dmdc.osd.mil/self_service  or http://www.cac.mil/
  2. Marriage Certificate (Original, with file number).
  3. Spouse's Social Security Card (Original, not laminated). 

*Note* If a social security card has not been issued, a temporary ID number will be assigned. The dependent will receive a maximum of three (3), 90 DAY period, ID cards. Failure to provide a social security card within the three (3), 90 DAY periods will result in a Direct Care suspension through Tricare. 

 

  1. Original of any Final Divorce Decree, Annulment, or Death Certificate from previous marriage(s) for Sponsor and/or Spouse. 
  2. Two (2) valid, unexpired, forms of identification listed in the Federal Information Processing Standards 201-2, and one form MUST be a photo ID. 

Active Duty Service Members please note:  AN EXPIRED DRIVER’S LICENSE CANNOT BE USED FOR DEERS IDENTIFICATION PURPOSES.

*Note* ANY foreign document(s) MUST be translated in English by an authorized source. Military members can have documents translated from Military OneSource for FREE by calling (800) 342 – 9647.

*Note* Dual Active Duty spouses or 100% Disabled American Veterans will be added to each other’s DEERS record but will not receive additional benefits or a dependent ID card and will continue to utilize their own CAC.

 *Note* For Active Duty and Reserve Marines, please ensure you visit the IPAC to enroll dependent(s).

 

 

Child and/or Step-Child – DEERS Enrollment

To enroll a child or step-child in DEERS, the following are required: 

  1. Sponsor must be present OR a Power of Attorney must be presented and signed by the sponsor OR sponsor may nominate dependent on the Rapids Self-Service website: https://www.dmdc.osd.mil/self_service or http://www.cac.mil/ 
  2. Child/step-child’s Birth Certificate (Original, with file number).
  • Certificate of Live Birth /Hospital Proof of Birth is acceptable for newborns that are born within 90 DAYS.
  1. Child/step-child’s Social Security Card (Original, not laminated). If a social security card has not been issued, a temporary ID number will be assigned. The sponsor will have 90 DAYS to provide the actual social security card to any DEERS office. Failure to do so will result in a Direct Care suspension through Tricare.  In order to avoid future Tricare issues, it is recommended that the child’s original birth certificate and SSN card be brought in to scan into DEERS during ID card renewals.
  2. Child/step-child aged 18 and over – Two (2) valid, unexpired, forms of identification listed in the   Federal Information Processing Standards 201-2, and one form MUST be a photo ID.

Step-child – Sponsor and spouse’s marriage certificate to verify relationship (Original, with file number).

CHILD OF A SINGLE MALE SPONSOR AT THE TIME OF BIRTH:

  • Birth Certificate with both parents’ names listed
  • Court Order (Judicial Determination of Paternity) OR a NOTARIZED Voluntary Acknowledgment of Paternity (***VAP) with both parents’ signatures, OR an Approved Dependency and Residency Determination (MUST reflect that over 50% support is provided)
  • VAP MUST contain respective state’s seal   

***We can only cross-service initial enrollments of illegitimate children if you possess a Court Order establishing paternity

We CANNOT accept/cross-service a Voluntary Acknowledgement of Paternity (VAP) from other services***

*Note* Children who are married or joined active military service are no longer eligible for benefits under the sponsor.

Ages 21 to 25 Child and/or Step-Child– ID Issuance/Renewal

***NEW*** PLEASE NOTE:  IF A DEPENDENT’S ID CARD IS LOST OR STOLEN, SPONSOR MUST INDICATE THIS IN BLOCK 21 OF DD FORM 1172-2.

College Student (Age 21 to 22)  

  1. Every ID issuance requires that the sponsor is present OR a Power of Attorney must be presented and signed by the sponsor OR sponsor may complete the DD Form 1172-2 with an official notary OR the sponsor may complete the DD Form 1172-2 on the Rapids Self-Service website:  https://www.dmdc.osd.mil/self_service or http://www.cac.mil/THE SPONSOR MUST INCLUDE STATEMENT THAT “OVER 50% SUPPORT PROVIDED” in block 21 of the DD Form 1172-2, or we will not be able to issue card.  A dependent ID card will be issued for four (4) years or less and can be renewed within 90 DAYS prior to the expiration date. 
  2. Child/step-child – Two (2) valid, unexpired, forms of identification listed in the Federal Information Processing Standards 201-2, and one form MUST be a photo ID. 
  3. A letter with semester dates, and anticipated graduation date, from the school registrar’s office certifying full-time enrollment OR Certification from The National Student Clearing House proving full-time enrollment. (MUST be enrolled full-time at one college).   
  • College MUST be an accredited school leading towards a degree of higher education and the sponsor MUST provide over 50% support.
  • If the student is enrolled in the FINAL semester prior to graduation AND the classes are required for graduation, then the student may carry less than a full-time load.
  • ID card will be issued to graduation date, if stated on the school letter.  If anticipated graduation date is not stated, ID Card will be issued to the day before 23RD birthday.

Tricare Young Adult (TYA) (Age 21 up to age 25) 

  1. Tricare Young Adult (TYA) MUST be purchased through Tricare prior to visiting the DEERS office.
  2. Every ID issuance requires that the sponsor is present OR a Power of Attorney must be presented and signed by the sponsor OR sponsor may complete the DD Form 1172-2 with an official notary OR the sponsor may complete the DD Form 1172-2 on the Rapids Self-Service website https://www.dmdc.osd.mil/self_service or http://www.cac.mil/.  A dependent ID card will be issued for four (4) years or less and can be renewed within 90 DAYS prior to the expiration date. 
  3. Child/step-child – Two (2) valid, unexpired, forms of identification listed in the Federal Information Processing Standards 201-2, and one form MUST be a photo ID.
  • Tricare Young Adult (TYA) MUST reflect in DEERS and be effective at the time of the ID issuance. o *Note* If DEERS does not reflect that Tricare Young adult is effective, our system will physically not allow us to generate/create the dependent ID card.

Parent/Parent In-Law – DEERS Enrollment

Every ID issuance requires that the sponsor is present OR a Power of Attorney must be presented and signed by the sponsor OR sponsor may complete the DD Form 1172-2 with an official notary OR the sponsor may complete the DD Form 1172-2 on the Rapids Self-Service website:  https://www.dmdc.osd.mil/self_service or http://www.cac.mil/.  A dependent ID card will be issued for four (4) years or less and can be renewed within 90 DAYS prior to the expiration date. 
  2. Approval of Dependency Determination (DD 1172-2) from Service Branch Project Office OR Defense Finance Accounting Services (DFAS) OR Original Court Order – See below.
  3. Parent/Parent In-Law’s Birth Certificate (Original, with file number). 
  4. Parent/Parent In-Law’s Social Security Card (Original, not laminated).

*Note* If a social security card has not been issued, a temporary ID number will be assigned. The dependent will receive a maximum of three (3), 90 DAY period, ID cards. Failure to provide a social security card within the three (3), 90 DAY periods will result in a Direct Care suspension through Tricare and the denial of a new ID card.

  1. Parent/Parent In-Law’s – Two (2) valid, unexpired, forms of identification that are listed in the Federal Information Processing Standards 201-2, and one form MUST be a photo ID.
  2. Sponsor’s/Spouse’s Birth Certificate (Original, with file number) to prove relationship to Parent/Parent In-Law.
  3. Sponsor’s Marriage Certificate (Original, with file number) to prove relationship to Parent/Parent In-Law.
  4. Sponsor - Two (2) forms of valid unexpired identification, one form MUST be a photo ID.

Active Duty Service Members please note:  AN EXPIRED DRIVER’S LICENSE CANNOT BE USED FOR DEERS IDENTIFICATION PURPOSES.  

*Note* Initial Application – To apply for a Dependency Determination and eligibility requirements, please contact the appropriate branch of service listed below (Please note each branch of service and DoD agency may have varying eligibility requirements):

*Note* Recertification – The sponsor MUST have the Recertification of the Dependency Determination from DFAS every four (4) years or less. The sponsor may submit the application for Recertification of the Dependency Determination within 90 days of expiration.  Parents and parents’ in-law aged 75+ will NOT receive a “FINAL” ID card.  It will be issued for 4 years.

This is the DoD Instruction DoD 1000.13-V2 Enclosure 8 regarding 100% DAV (Disabled American Veterans) and their dependents to be able to obtain a DoD ID card for access to Exchange’s, Commissary and MWR facilities. Some base ID offices are unaware of this instruction, especially Guard or Reserve ID facilities.

In the Instruction “C” is “Commissary”, “E” is “Exchange” and MWR is “Morale, Welfare and Recreation”.
